1. Fill up the application form

2. Submit two (2) 2x2 ID pictures

3. Pay PhP100.00 membership fee

4. Attend at least 3 Malta missions held every Saturday throughout the year

   (Philippine Association Sovereign Military Order of Malta located in UN

   Avenue, Manila).

5. Attend at least 2 minor medical and/or surgical missions.

6. Take the exam to be given during the General Assembly (within the

   year).

7. Accumulate at least 80 points by  participating in MMI activities (e.g.,

2. Submit two (2) 1x1 ID pictures

3. Pay PhP80.00 membership fee

4. Accumulate 80 points by:

   a. participating in MMI activities

   b. working at the MMI office.

     One hour of work = 5 points.

   c. donating a canister which will be used as container for tools that will

     be autoclaved.

     One canister = 5 points.

   d. donating 5 pieces of manila paper = 5 points.

5. Attend at least one of the ff:

   Malta Minor Medical Mission

   Tondo Minor Medical Mission

   Emmaus (Home for the Aged)

   Bethany (Orphanage)

6. Take the written exam
